New Patients

Registrations are welcome from new patients who live within the practice catchment area. Please check by entering your home postcode to see if it falls within our area. We are unable to accept new patients who live outside of our catchment area. You can find an alternative surgery near you here.

Moving out of area?

If you are currently registered with us but are moving please check to see if you will still be in our catchment area. If you are moving out of the catchment you will need to register with a new GP surgery.

Why can't you keep me on?

Modern Primary Care is an integrated team of health professionals who work closely together to provide coordinated care to a local population. It is not possible to provide a high quality and efficient service to patients outside of our area. We are unable to make exceptions. You can find GP surgeries near to you here.

Registration Process

You do not have to provide ID when registering with a GP, but it is helpful to do so to speed up the registration process and obtain your previous GP records. The following suggested documents can be used as ID at a GP: Passport, Birth certificate, Travel document, HC2 certificate, Utility bill.

A registration form (GMS1) must be completed for each new patient. This can be downloaded and printed off by clicking the link. Forms are also available at reception. You will also be asked to fill in a short medical questionnaire as there can sometimes be a delay in obtaining your medical records from your previous surgery.

Your registration will be processed as quickly as possible but please allow 2 working days for this to be completed.
