New Patients

Registrations are welcome from new patients who live within the practice catchment area. Please check using the web tool below to see if your home postcode is included. We are unable to accept new patients who live outside of our catchment area.

*** Update 01/04/19: unfortunately NHS Digital have closed the website that allowed access to the practice catchment area web tool. We are awaiting instructions as to how to regain access to this. We apologise for the inconvenience. Please speak to reception in the meantime who will be able to advise you as to whether your home address lies within our catchment area. ***

Registration Process

All patients are required to produce two forms of identification to prove eligibility for NHS Services. These include photographic ID or birth certificate and proof of your current address. You will need to bring these to the surgery in person.

A registration form (GMS1) must be completed for each new patient. This can be downloaded and printed off by clicking the link. Forms are also available at reception. You will also be asked to fill in a short medical questionnaire as there can sometimes be a delay in obtaining your medical records from your previous surgery.

Your registration will be processed as quickly as possible but please allow 2 working days for this to be completed.
